Tran Thanh (preferred name Van) is an experienced and compassionate Care Plan Nurse who plays a vital role in helping patients manage their ongoing health needs. Van works closely with your GP to coordinate your care, review your health goals, and ensure you receive the right support from allied health professionals.

A Chronic Condition Management Plan is a personalised care plan covered by Medicare for patients with a chronic or terminal medical condition expected to last six months or longer.

The plan outlines your health goals and, if required, provides access to allied health professionals such as physiotherapists, dietitians, or psychologists to support your ongoing care.

Please note that Medicare does not cover GP consultations on the same day you see the Care Plan Nurse. If you choose to see your doctor on the same day for an unrelated matter, a private fee will apply.

To ensure your care plan can be completed, please book your care plan nurse appointment on a day when your GP is on duty, as your GP must provide the final review of your plan.
If your GP is not working on the day of your appointment, your care plan cannot be finalised, and you will need to return on another day to see your GP in person.
